Proposed federal budget cuts to Medicaid are poised to devastate Washington, D.C.’s healthcare system, particularly affecting hospitals that serve the city’s most vulnerable populations. The Republican-led House recently passed a budget resolution aiming to reduce spending by $2 trillion over the next decade, with $880 billion in cuts targeting programs under the House Committee on Energy and Commerce, which oversees Medicaid.

These reductions could slash the Federal Medical Assistance Percentage (FMAP) for D.C. from 70% to 50%, aligning it with states like Maryland and Virginia. Such a decrease would result in a significant loss of revenue for local healthcare providers. For instance, Howard University Hospital, where approximately 40% of patients are covered by Medicaid, could face an annual revenue reduction of about $33 million. This financial strain is deemed “untenable” by hospital leadership.

The potential impact extends beyond individual institutions. Citywide, hospitals could collectively lose over $464 million annually if these Medicaid cuts are implemented. This financial blow would likely force hospitals to reduce or eliminate critical services, including trauma care, maternity services, and behavioral health programs. Such cutbacks would not only affect Medicaid recipients but also the broader patient population, leading to longer wait times and diminished access to essential healthcare services.

Community health centers, which rely heavily on federal funding, are also at risk. These centers serve a substantial portion of D.C.’s residents, providing primary care to those who might otherwise lack access. Funding cuts could severely limit their capacity to offer necessary services, further straining the healthcare safety net.

The proposed Medicaid cuts are part of a broader budget plan that includes extending tax breaks and increasing funding for defense and border security. While aiming to reduce federal spending, these cuts could disproportionately harm low-income and underserved communities, exacerbating existing health disparities.
